SandwYch is a comprehensive digital healthcare platform developed to transform the way hospitals, medical professionals, patients, and caregivers engage throughout the continuum of care. Built for a U.S.-based healthtech client, the system addresses challenges around fragmented hospital workflows, lack of post-discharge support, and low caregiver engagement. SandwYch brings together multiple stakeholders through a modular, interoperable ecosystem designed to deliver round-the-clock, personalized, and compliant patient care.

The platform includes a responsive web interface and a mobile app, with dedicated modules for hospitals, system admins, care professionals, patients, and caregivers. Integration with leading third-party healthcare APIs such as Epic, FDB, and Surescripts ensures reliable and secure access to critical patient data, prescriptions, and medication safety insights.

Background / The Challenge

Modern healthcare delivery often suffers from disconnected workflows, manual communication, and limited visibility across teams and phases of care. Hospitals lack centralized systems to track patients, assign care teams, and validate prescriptions in real-time. Moreover, operational inefficiencies lead to delays and prevent care teams from making data-driven decisions promptly.

Equally pressing is the gap in post-hospitalization support. Patients and their caregivers frequently navigate the recovery phase alone, facing emotional, informational, and logistical challenges. Without clear care plans, access to resources, or real-time feedback loops, care partners remain under-supported—despite playing a crucial role in recovery outcomes.

The client envisioned a solution that would not only streamline operations within hospitals but also extend support and empowerment to patients and their care partners. SandwYch was built to deliver that vision.

The Solution

SandwYch was developed as a five-module digital ecosystem that ensures care continuity from hospital admission to recovery, combining patient data management, role-based collaboration, public engagement, and real-time caregiver interaction. Its core components include:

  1. Marketing Website (Public Engagement & Subscription Entry Point)

    • Serves as the platform’s public face for onboarding hospitals and sharing caregiving insights.
    • Enables subscription management, payments, and renewal tracking.
    • Hosts blog content, caregiver surveys, and health awareness articles.
  2. Admin Portal (Platform Management)

    • Allows system administrators to track hospital subscriptions, manage user roles, and configure system-wide settings.
    • Provides visibility into usage patterns, activity metrics, and survey data.
  3. Client Portal / Hospital Dashboard

    • Lets hospitals manage patient records, assign care teams, and coordinate services.
    • Integrates with Epic for retrieving patient demographics, medical history, and medication data.
    • Supports dynamic care team management and secure role-based access.
  4. Medical Professional Portal

    • Role-specific interface for doctors and nurses to monitor treatment progress, prescribe medications (integrated with FDB for drug safety checks), and collaborate on care delivery.
  5. Mobile App for Patients & Caregivers

    • Patients can view care plans, schedules, alerts, and request support resources.
    • Caregivers receive task lists, patient updates, and can log mood, medications, and daily activities.
    • Real-time communication allows both sides to stay informed and connected.
